Overview:
Provide case management, supportive counseling, and advocacy
services to elderly clients or persons with disabilities
to help enhance their well-being and independence. Works
independently using supervision and consultation as appropriate
while maintaining considerable latitude for the use of initiative
and independent judgment.

LSW $12,418 - $13,982 ($11.94
- $13.44 per hour) MSW $16,004 - $18,433 ($15.39 - $17.72
per hour)
Qualifications:
- Bachelor's degree in Social Work
- LSW licensure with the MN Board
of Social Work
- Two years experience min medical
social work/case management
- One year experience working with
elderly and or people with disabilities
- Ability to communicate effectively
and represent Neighbor to Neighbor professionally with
clients, staff, supervisor and community at large
- Valid driver's license
- Competency in electronic recordkeeping
